Cancer is a disease of genes that gone out of control. Certain genes that direct the orderly replication of cells have malfunctioned and cause the cell to divide unabated. Eventually, these damaged cells spread into neighboring cells of other tissues and multiply all over the body. All cancer is genetic in origin, that is, it is initially set off by altered genes. However, just a tiny portion of cancer is inherited: a mutation carried in reproductive cells, passed on from one generation to the next, and present in cells throughout the body. Most cancers come from random mutations that develop in body cells during one's lifetime – either as a mistake when cells are going through cell division or in response to injuries from environmental agents such as radiation or chemicals. Cancer usually arises in a single cell. The cell's progress from normal to malignant to metastatic appears to follow a series of distinct steps, each one controlled by a different gene or set of genes. Several types of genes have been implicated. Oncogenes normally encourage cell growth; when mutated or overexpressed, they can flood cells with signals to keep on dividing. Tumor-suppressor genes normally restrain cell growth; when missing or inactivated by a mutation, they allow cells to grow and divide uncontrollably. (The inherited genes that predispose for breast and ovarian cancer, Li-Fraumeni syndrome, retinoblastoma, Wilms' tumor, and familial adenomatous polyposis are malfunctioning tumor-suppressor genes.) DNA repair genes appear to trigger cancer – and perhaps other inherited disorders – not by spurring cell growth but by failing to correct mistakes that occur as DNA copies itself, letting mutations accumulate at thousands of sites. (Genes that have been linked to hereditary colon cancer are such â€Å"proofreader† genes.)   

Collaborative Practice in Health Care Essay

Collaborative practice in health care occurs when a member of the health care team consults with another member to provide patient care. Collaboration most often occurs between doctors and nurses. â€Å"Collaboration is defined as a relationship of interdependence; the ability to work together involves trust and respect not only of each other but of the work and perspectives each contributes to the care of the patient† (Phipps and Schaag, 1995, p. 19). Effective collaborative practice amongst all health care team members leads to continuity of care, professional interdependence, quality care and patient satisfaction and decreased costs. Ongoing collaboration between health care members results in mutual respect, trust and an appreciation of what each individual brings to the overall goal in rendering care to the client. The following vignette will provide the foundation for the discussion of collaborative care, differentiating between nursing diagnosis and collaborative proble ms, and potential barriers to successful collaboration. JG is a 74 year old married Hispanic male diagnosed with colon cancer. He had a history of prosthesis placement of his left lower leg; he is ambulatory. He is a diabetic on oral medications. He worked as a farm laborer. He lives with his wife she does not speak English she is a homemaker. He has a son who lives nearby and a nephew who periodically visits him. JG can understand some English. He does have some difficulty expressing his health concerns to the staff because of his limited vocabulary. His son or nephew brings JG to his clinic appointments. He receives weekly chemotherapy at the outpatient oncology clinic. The day I cared for JG he arrived at the clinic accompanied by his nephew. This was week seven of his treatment. His clothing was dirty, he smelled of stool, his fingernails were dirty, hair uncombed, he appeared to be dehydrated. He reported bowel movements of eight stools per day with complaints of occasional abdominal cramping. He denied nausea or loss of appetite. He stated that he was very tired and was not able to do much at home. His main concern was the frequency of his bowel movements. He reports having to go to the bathroom two to three times during the night and has episodes of soiling the bed. He reports that sometimes he does not feel the  urge to go. JG was wearing adult diapers. He expressed concern that it was getting expensive for him to purchase. The nephew confirmed that JG toileting has created a problem in the home. His nephew verbalized that JG had medication for diarrhea but ran out of it and he did not have the money to purchase the medication. When questioned why he was using a wheelchair he stated that his foot hurt to walk the distance from the lobby to the treatment room. He mentioned that it was probably due to an ingrown toe nail. He also asked how he could obtain a wheel chair for his personal use at home. Physical assessment revealed that he had a necrotic area on the ball of his left foot with surrounding redness, lost 12 pounds in six weeks, poor skin turgor, hyperactive bowel sou nds, and his blood pressure was slightly lower than baseline. In the ambulatory chemotherapy setting, the clients do not always see their physician every time they receive treatment. The nurse must ascertain when to collaborate with the physician on issues regarding the patients status, response to treatment, or toxicities that may be life threatening. It is essential that the nurse is capable to communicate effectively her-(Be careful with gender bias, nurses come in both genders.) observations to the physician. Collaborative problems are detected from the nurse’s assessment of the patient. The nurse’s monitoring of the patient status is to evaluate physiological complications that may threaten the patient’s integrity. Management of collaborative problems will include implementing physician prescribed and nurse prescribed actions to curtail escalation of the problem and preventing patient harm. From the nurse’s assessment, she also formulates a nursing diagnosis. The nursing diagnoses are stated in the form of the problem, the etiology and the symptoms that the nurse observes. Nursing diagnosis can include a current or potential problem, an at risk problem, or a wellness diagnosis. Nursing diagnosis provides the framework from which the nurse begins to devise a plan of care and nursing interventions. In the case of JG, there were two collaborative problems identified. Two  problems I collaborated with physician, these were: 1. JG is experiencing toxicity from the chemotherapy. There is potential for electrolyte imbalance, circulatory collapse. 2. The necrotic area on his foot was a new development in his condition. There is potential complication for infection The collaborative problems discussed with JG physician and nurse quickly resolved. JG did not receive his chemotherapy. He was given an injection of sandostatin LR to help minimize his diarrhea; a stat basic metabolic panel was obtained; and he was given intravenous hydration with potassium. The doctor made a referral to JG podiatrist for the next day to assess the integrity of his left foot. Listed are four, but not all, possible nursing diagnosis obtained from my assessment. 1. Diarrhea related to chemotherapy manifested by hyperactive bowel sounds and eight loose stools. 2. Bowel incontinence related to loss of rectal sphincter control and chemotherapy manifested by fecal odor, fecal staining of clothing, urgency. 3. Altered Nutrition related to colon cancer manifested by diarrhea, abdominal cramping. 4.Ineffective management of therapeutic regimen related to JG lack of knowledge of his disease manifested by his inability and unwillingness to manage his symptoms. Considering JG comments regarding his finances, his overall physical appearance and the comments from his nephew, I decided to consult with the social worker. I felt that a home visit or a thorough investigation of JG home situation was warranted. The social worker was able to arrange for in home support, and helping the patient with insurance issues so he could obtain the needed supplies. I did not think to enlist the participation of the dietician. In retrospect, the dietician would have been a valuable resource to assess JG caloric intake and recommendations for optimal nutrition. I felt that the above incident demonstrated collaboration amongst health care providers. The physician in this case was receptive to the nurse’s observations with respect to her capabilities of accurate assessment of the patient’s condition and potential complications. This is not always the case, barriers to collaboration are also inherent in the health care industry. Barriers occur in patient situations where the physician is not sympathetic or does not trust the nurse’s evaluation of patient condition. The nurse may have feelings of inferiority, lack of confidence and does not appropriately collaborate with the physician correct information. Conflicts in the goals desired for the patient is often cited as a barrier to collaboration. I recall an incident of a male patient diagnosed with metastatic breast cancer. His appearance was that of an individual who had been in a Nazi concentration camp. The nurse wondered why the physician was treating this man aggressively. In her mind, this patient was not an appropriate candidate to receive the particular treatment that was ordered. She feared the patient would not tolerate such an aggressive schedule and that it was pointless to put this poor man through treatment. The patient was diagnosed two years ago. He is still receiving treatments, he has gained weight and in October of last year he hiked to the summit of Mt. Whitney. Role conflict is another major barrier to collaboration. To deliver cost effective care, many institutions utilize nurse practitioners and physician assistants. Role conflict arises when practitioners have opposing views or expectations (Blais, Hayes, Kozier, & Erb, 2002). Role conflict and can lead to litigation. According to Resnick, physicians hesitate to collaborate informally with Nurse Practitioners for fear of being held liable for the actions of the Nurse Practitioner (Resnick, 2004). Clear definition of roles  for practitioners is essential to prevent misunderstanding. In conclusion, collaborative practice is the gold standard that health care practioners should strive towards. The nurse is central in determining the patient issues that warrant collaboration and she must be able to effectively communicate her observations. Collaborative practice minimizes complications that could lead to tragic outcomes. Freedom of the Press Versus Right to Privacy

Privacy has become a big issue in contemporary jurisprudence. The â€Å"right to privacy† is enshrined in the United Nations Declaration of Human Rights, and guaranteed by Article 8 of the European Convention on Human Rights. But Article 8 is balanced by Article 10, which guarantees â€Å"free expression of opinion†. So what right has priority when they conflict? Under what circumstances, for example, is it right to curtail press freedom in order to protect the right to privacy, or vice versa? The same balance is being sought between the right of citizens to data privacy and government demands for access to personal information to fight crime, terrorism, and so on. Freedom of speech is a fundamental democratic liberty. It is a necessary protection against abuses of power and cover-ups of wrongdoing by public officials. It was never more effectively displayed than in the Watergate investigation, which brought down Richard Nixon in 1974. But one can have too much press freedom. Over the years, the tabloid press has become increasingly intrusive, claiming the right not just to expose corruption and incompetence in high places, but to titillate readers with scandalous revelations about the private lives of the famous. What started off as entertaining gossip about royalty and film stars has burgeoned into a massive assault on privacy, with newspapers claiming that any attempt to keep them out of the bedroom is an assault on free speech. The issue has just been tested in Britain's High Court. In March, Britain's leading scandal sheet, The News of the World, published an â€Å"exclusive† front page story, under the headline â€Å"F1 Boss Has Sick Nazi Orgy With 5 Hookers†. It told how Max Mosley, President of the Federation Internationale de l'Automobile (FIA, the body that oversees world motoring and racing) and son of the former British fascist leader, Sir Oswald Mosley, had, two days earlier, taken part in a sadomasochistic â€Å"orgy† with a â€Å"Nazi theme† in a private apartment in London. The story was accompanied by photographs taken clandestinely by one of the women in cooperation with the News of the World, which readers were invited to download from the paper's website. Max Mosley admitted participating in this (not illegal) happening, but sued the News of the World for breach of privacy; the newspaper argued that it was in the â€Å"public interest† that Mosley's sexual activities be disclosed. The presiding judge, Justice Eady, rejected the newspaper's defense, and awarded Max Mosley 60,000 English pounds ($115,000) compensation for the invasion of his privacy, the highest damages so far given for a complaint brought under Article 8. There is a curious aspect to Eady's judgment. He rejected the News of the World's â€Å"public interest† defense, because he found no evidence that the sadomasochistic party had a â€Å"Nazi theme†. This implies that had there been a Nazi theme, it could have been legitimate to publish it, given Mosley's position as FIA president. But surely the particular nature of Mosley's private fantasies is irrelevant to the case. It is hard to see why I am less entitled to privacy because I am turned on by a Nazi uniform than I would be if I were excited by a pair of knickers. What Eady's judgment did accomplish was to highlight the crucial distinction, necessary for all clear thinking about privacy, between what interests the public and what is in the public interest. So how can this distinction be made effective? France has a privacy law that explicitly defines both the scope of privacy and the circumstances in which the law applies. By contrast, in Britain it is left to judges to decide what the â€Å"right to privacy† means. There is a natural fear that specific legislation designed to protect privacy would muzzle legitimate press inquiries. At the same time, it is widely acknowledged (except by most editors and journalists) that a great deal of media intrusion is simply an abuse of press freedom, with the sole aim of boosting circulation by feeding public prurience. A law that curtails the abuse of press power while protecting its freedom to expose the abuse of political power would be difficult, but not impossible, to frame. The essential principle is that the media should not be allowed to pander to the public's prurience under cover of protecting the public interest. What famous people – indeed ordinary people, too – do in private should be off limits to the media unless they give permission for those activities to be reported, photographed, or filmed. The only exceptions would be if a newspaper has reasonable grounds for believing that the individuals concerned are breaking the law, or that, even if they are not breaking the law, they are behaving in such a way as to render them unfit to perform the duties expected of them. Thus, a pop star's consumption of illegal drugs may be reported, but not his or her sexual habits (if they are legal). The private life of a politician may be revealed if it is expected to have consequences for the way the country is being governed; that of a top executive of a public company if it may affect the returns to shareholders. This should be the only â€Å"public interest† defense available to a media outlet that is sued for invasion of privacy. The media might become a bit drearier, but public life would be far healthier. Aristotle Virtue Ethics Essay One basic notion in Aristotelian ethics that occupies a central significance is Aristotle’s belief in the role of man’s activities in order for one to acquire ethical knowledge. That is, for one to become virtuous or to obtain virtues one should not merely confine himself to mere studying of these virtues but rather one should, more importantly, actualize this knowledge of the virtues. Thus, for one to become good, one should do good. Aristotle further stretches his ideas by proposing the doctrine of the mean. The essence of this doctrine dwells on the basic precept that one ought to avoid the extremes and, instead, settle for the â€Å"mean†. The actions of men, more specifically, ought to be framed upon the â€Å"mean† which is the virtue. For example, the virtue of courage rests on the mean between two extremes: cowardice or the â€Å"lack† of courage, and rashness or the â€Å"excess† in courage. It can easily be observed that Aristotle suggests that one should live a life that does not border on the things that are on the â€Å"most† and the â€Å"least† levels. For example, if I were to follow Aristotle’s ethics I should always see to it that I should consistently draw myself towards the middle value and avoid slanting towards cowardice and excess. All this could have been very well except for one small thing that I cannot seem to fully comprehend. How is it possible for one to ascertain that one is actually taking the middle path? Or how is it possible for one to know that this or that is the middle value or is the virtuous action? Perhaps the key in having an understanding to the notion of the â€Å"mean† is that one should act. That is, as I continually have these ideas as to what I must do when faced with an ethical situation or, at the least, an ordinary situation, I should nonetheless take the course of action so that I will be able to obtain a qualitative understanding that what I am doing is the â€Å"excess†. Aristotle’s virtue ethics reminds me of Plato’s conception of ethics. For the most part of the Republic, Plato attempts to arrive at a conception of a just life by centering on the notion that the just life or that which is good is better than living a life molded on an evil framework and one which prompts individuals to act in an evil manner. At the onset of the Republic’s Book II, a conception of the idea of â€Å"justice† is advanced as the working of an individual in accordance to the role in which one is best suited as well as the belief for non-interference in the activities of others. In essence, this principle is closely related to Plato’s perspective on acting in accordance to one’s nature or intrinsic being which results to the state or condition of being â€Å"just† or acting justly once the individual acts in line to his very nature. If I were to choose which ethical precept would be better or would fit me best, I would rather be inclined to adopt Aristotle’s virtue ethics over the other because it offers me a chance to actualize myself through my actions and be guided accordingly. Whereas for Plato, what I am seeing is that I should get to know my self first before I act so that I can be good. But this cannot be met easily essentially because I find it quite difficult to know myself if I would not act first. In the light, if all the students in a certain class would build up a virtue like that of Aristotle’s point of view, the achievement of an environment which is formidably that built on the concept of philosophical and tremendously beneficial notions in life, the attainment of the virtue of goodness is towering in the highest hopes—although it eventually does not also undermine the concept of realism. Gender Differences Within The Education System The Education System consists of all school types, from pre-schools to universities all over the world. There are many different types of gender differences within the education system including: academic, social, emotional and behavioural. Academic gender differences are one of the most highly researched topics within the educational system. Within this essay I will discuss the widening gap between gender and achievement, and try to explain why these differences may be apparent, and how to change them. Research shows that academic gender differences are evident from 3years of age. Statistics from the Foundation Stage Profile Results for England for 2007-08 show that girls outperform boys in all 13 assessment scales. The main gap areas are social development (10% difference between genders) emotional development (11%) linking sounds and letters (11%) writing (18%) reading (11%) creative development (14%) (Steve Maynard, Gender differences in school, 2008) During the 70s when academic achievement was being investigated research was showing that girls where underachieving, they did less well in GCE exams, left school earlier than boys and were less likely to go to university. The main view was that gender differences were natural and unalterable, known as the Biological Deterministic approach. This may suggest why boys and girls were treated differently within the education system, as their adult roles were different I.e. men go out to work and women are home-makers, therefore not needing qualifications. However, if this is the case it is important to address why research within the past decade, evidently shows an increasing gender achievement gap with male students lagging behind females on a number of important indicators of school success. (Clark. M, Flower. K, Walton. J, Oakley. E., 2008). It could be argued that this is down to the changing views of society, along with the introduction of a compulsory national curriculum where both girls and boys where required to take foreign languages, English, sciences and maths. According to Wilkinson (1994) the Genderquake has a major part to play in the success of women within education today. He states that fundamental changes in attitudes towards female roles in society, have lead women to have a more positive attitude towards education as a means of improving their chances of success at work. A report from the Rowntree Foundation (2007) found that academic gender differences can continue up to age 16, showing that boys outnumber girls by 20% as low achievers at GCSE. Van Houtte (2004) suggests this may be due to the fact that males have a less study orientated culture that females. This would explain why achievement is higher in vocational and technical courses such as business studies, IT, geography (where more boys enter) and in separate sciences a more hands on approach to learning in males than females. Berg and Klinger (2009) found that subject performance is often associated with gender-specific stereotypes, and self-perceptions such as reading for girls and maths for boys. This may explain why one of the only academic subjects males seem to have out performed girls in is mathematics. Many studies show that mathematical achievement is a male dominated subject. A study in 2008, (Van de gaer, Pustjens, Van Damme. De Munter.) amongst Flemish students showed that boys performance in maths is linked to their participation within that subject boys participate in maths classes more than girls. Further support for the above points comes from (Kyong Hee Chee, 2005) whose analysis indicated that women are more likely to possess an academic ethic than men. They also found that women tend to have higher Grade Point Averages (GPAs). They found GPAs to be positively associated with active participation in extra-curricular groups and clubs, and GPA was negatively related to employment for men. These results pose the question in why there is such a large gap in achievement between genders? Experts say that the reason for this may be down to the difference in nature and nurturing between girls and boys. Genetic theories state that females excel in language based subjects due to their greater verbal and reasoning abilities, where as males have a high level of innate spatial ability, increasing their understanding of shape and form. As girls start to talk, read (read more often) and develop fine motor skills earlier than boys, they also progress quicker and develop more positive attitudes, they are generally better equipped for the start of formal schooling than boys. Once in school, girls tend to be more task-focused more methodical and conscientious, and far better at tackling coursework. Boys tend to be action-oriented impatient, imaginative, and inclined to take risks. They develop fine motor skills later than girls, and their learning and motivational skills are different. (Duffy, M. 2002) There have been many discussions about how to close the gap between gender and achievement. This has included changing the ways that children learn, and it has been suggested that more male teachers in primary schools could help boys within their achievement. It has been argued that women teachers talk to much causing boys to switch-off. Celia Lashlie, author of Hell Be Okay states that women teachers need to talk in lower pitches when teaching and use more non-verbal cues like males do. Male can be seen as positive role models towards education teachers can break down assumptions such as reading is for girls.(Times Educational Supplement) There are many campaigns in place to encourage boys to read, such as Welsh rugby players promoting reading within schools and libraries. Maloney (2002) states that boys like to read books that reflect themselves and who they aspire to be and appeal to their sense of humour. Interestingly,boys enjoy looking at newspapers, magazines and comic books but do not consider this reading, as these materials arent valued in school. It is therefore important that to encourage reading and literacy skills, schools need to provide enough material for boys to want to read. The most recent idea, that supports previous research findings is too change assessments to suit each of the genders needs. AQA, the UKs largest school exam board, propose developing gender-specific alternatives, that are tailor made for girls and boys, to GCSEs. It has been suggested that these new key-stage 4 qualifications in English, Maths and Science could be taught as early as 2011, with coursework options for girls and more traditional exams aimed at boys. AQAs director of curriculum and assessment, said: We could offer a route for boys that is very different to a route for girls. Girls tend to perform better with coursework while boys do better with end-of-year exams. So we are pursuing that in science to see if we could have an option in science where we might have a straightforward examination for boys but a possibility of having a coursework option for girls. Even though the research within this essay often has a large sample size, and seems to have the same effect across cultures, It has been argued that, although research has shown that by the age of seven, some boys are almost two and a half years behind their brightest female class mates, gender is only a small part of academic gender gap, and factors such as poverty, ethnicity and birth season have a larger effect on a childs academic achievement. The analysis conducted in 2000, revealed that the most disadvantaged pupils are male from a poor, ethnic-minority background, born in the summer, never went to nursery and spent their primary years moving from school to school. These children were more than two years behind more socially-advantaged, winter-born, female classmates. (Birminghams Education Authority) Also, biological theories for gender differences within education, can be strongly criticised. Genetic explanation can not explain how gender differences have narrowed between mathematics and science based subjects since the 1980s. Arguably, if these differences were genetic they would be expected to remain constant. Kelly (1982) suggests that the types of toys children play with can be attributed to the differences in spatiality ability. Further support for this point comes from Sharpe (1976), who argues that childhood socialization plays a large part in masculine and feminine identity roles. This may suggest why women tended to stick to feminine subjects such as home economics, and art other than science and technology, which are seen masculine. This provides further support for the views of the Gender-quake and the changing roles of women within society, henceforth the success of females academic achievement over males. In conclusion, there are many reasons for academic gender differences within the education system, including the stereotypes and the views of gender roles within society. In order to close the gap that seems to be continuously growing, changing ways in which teaching is approached seems to be a logical solution. This includes the encouraging boys to read, as it has been suggested that due to a lack of reading boys are held back in their writing skills. changing the ways in which assessment is carried out, may be more beneficial. Instead, they have to aware what their judgments are and â€Å"then holding them lightly so they can still hear what others are saying† (Gerard and Ellinor 2001, p. 7). After listening carefully to other’s ideas, they need to reflect their own assumptions. Therefore, to revising whether those assumptions are linked to the organization or not. If they cannot understand the differences, they must inquire for more information. Hence, this process of dialogue enables each employee to foster different views and converge them together to become one unique aim. Organizational culture is enhanced. In agreement with Gerard and Ellinor, Isaacs (1999) analyzed four principles of dialogue based on Bohm’s research in 1996. They are listening, respecting, suspending and voicing. Firstly, Isaacs had compared listening skill in dialogue to listening to music. He stated a single note of music could not deliver the meaning of the whole song. It is similar to one’s role in a conversation. A single idea is not sufficient to set purposes and  cultures for the whole organization. Hence, dialogue is an excellent practice to give people a chance to listen deeply and get into the nature of the conversation. Secondly, he defined respecting as getting to know more about one person and figure out what sources or circumstances has created their particular thinking. Based on this understanding, people in an organization will pay more respect to each other. The main goal of respecting in dialogue is not to seek decision but to tolerate difference, gap and conflict (Isaacs 1999). The third principle of dialogue, suspending, is determined similar to Gerard and Ellinor (2001). And the last principle Isaacs mentioned is voicing. He suggested people should listen internally so as to select what should say and what should not say in a circumstance. Sometimes keeping silence and listen can achieve the best result. Therefore, the purpose of voicing means people contribute their speech, not only for themselves, but to the whole idea. Overall, these principles are considered having positive effects on organizational learning. They emphasize group and organization achievements rather than an individual accomplishment. In Schein’s study (1993), he described dialogue as â€Å"talking around the campfire† (p. 391). He used â€Å"campfire† as a metaphor to explain how decision is made through dialogue. In the past, people sat around campfire during meeting and shared their own opinions. Arguments would never come up as people just simply expressed their thoughts without any discussion or debate. Through that, they were aware themselves which idea was acceptable and were unacceptable (Schein 1993). This process allowed enough time for each person to listen to a deeper layer of other’s opinions then reflect on their own assumptions. Moreover, Schein introduced the check-in concept. At the beginning of the meeting, each person will respectively contribute his or her ideas, views and feelings to the group as a whole, and therefore, â€Å"has helped to create the group† (Schein 1993, p. 392). Lastly, Schein stressed the limitation of eye contact. This makes people feel ea sier to suspend disagreements and concentrate on listening.
